Airway & Orthodontics: Understanding the Connection Between Breathing, Jaw Development, and Your Smile

When most people think about orthodontics, they think about braces, Invisalign®, and creating a beautiful smile. While a straight smile is an important part of orthodontic care, modern orthodontics looks beyond tooth alignment.

At Cui & Lin Orthodontics in Honolulu, we evaluate how the teeth, jaws, facial structures, and oral function work together to create a healthy, balanced smile.

A comprehensive orthodontic evaluation considers not only how teeth fit together, but also factors such as jaw development, facial growth, bite function, tongue space, and breathing patterns.

What Is the Airway?

The airway is the pathway that allows air to travel from the nose and mouth to the lungs. Healthy breathing depends on many interconnected structures, including the nasal passages, throat, tongue, jaws, and surrounding soft tissues.

Airway development can be influenced by many factors, including:

  • Jaw growth and development
  • Upper jaw width
  • Facial growth pattern
  • Tongue posture and function
  • Nasal anatomy
  • Tonsils and adenoids
  • Allergies and nasal breathing habits
  • Overall health and lifestyle factors

Because breathing is influenced by many different systems, airway concerns often require collaboration between orthodontists and other healthcare professionals, including ENT physicians, pediatricians, sleep specialists, and other medical providers.

Why Does an Orthodontist Evaluate the Airway?

Orthodontists specialize in the growth and development of the teeth and jaws. During an orthodontic consultation, we evaluate much more than crowded teeth or a misaligned smile.

At Cui & Lin Orthodontics, we assess:

  • Upper and lower jaw relationships
  • Dental arch development
  • Bite alignment
  • Facial balance and growth patterns
  • Tongue space
  • Oral habits such as thumb sucking or mouth breathing
  • The relationship between the teeth, jaws, and surrounding structures

Understanding these factors allows us to create treatment plans that are personalized to each patient’s unique anatomy.

Signs That May Benefit From an Airway Evaluation

The following signs do not necessarily mean a patient has an airway disorder, but they may indicate that a more comprehensive evaluation could be beneficial:

Children

  • Mouth breathing
  • Snoring during sleep
  • Restless sleep
  • Difficulty waking up
  • Trouble concentrating at school
  • Crowded or blocked-out teeth
  • Narrow upper jaw
  • Long facial growth pattern

Adults

  • Chronic mouth breathing
  • Poor sleep quality
  • Teeth grinding
  • Jaw discomfort
  • Difficulty maintaining nasal breathing
  • Concerns about facial balance or bite function

It is important to note that many people are not aware that they have breathing or airway issues if the symptoms are mild. A thorough evaluation helps determine whether orthodontic treatment alone is appropriate or whether collaboration with another healthcare provider may be recommended.

How We Evaluate Airway and Facial Development

At Cui & Lin Orthodontics, we combine advanced technology with a detailed clinical examination.

Your orthodontic evaluation may include:

Digital Photography

Allows us to evaluate facial balance, smile characteristics, and growth patterns.

Intraoral Scanning

Our digital scanner creates a detailed 3D model of your teeth without traditional impressions.

Facial Soft Tissue Scan and Analysis

Evaluates the relationship between facial structures, lips, and smile aesthetics. We are able to overlay this information onto your skeletal structure to see how soft tissue and hard tissue relate to each other.

3D CBCT Imaging

Three-dimensional imaging allows us to evaluate structures that cannot be fully visualized on traditional two-dimensional X-rays, including:

  • Jaw anatomy
  • Tooth positions
  • Sinuses
  • Bone structures
  • Airway-related anatomy

Our Vatech Green X 21 CBCT system features advanced low-dose imaging protocols, allowing us to customize imaging based on each patient’s specific diagnostic needs while following the ALARA principle (“As Low As Reasonably Achievable”). This new technology also allows us to capture much more data with less X-ray exposure than traditional sets of orthodontic X-rays.

Can Orthodontic Treatment Improve Breathing?

Orthodontic treatment may help improve certain structural factors that contribute to oral function, such as:

  • Expanding a narrow upper jaw when appropriate
  • Improving dental arch form
  • Creating additional tongue space
  • Correcting bite relationships

However, breathing is complex and can be influenced by many factors outside orthodontics.

Airway concerns may involve the nasal passages, soft tissues, sleep patterns, and overall health. For this reason, orthodontic treatment is often one part of a multidisciplinary approach.

If additional evaluation is appropriate, we may work together with your physician, ENT specialist, or sleep medicine provider.

Orthodontic treatment alone is not a definitive treatment for obstructive sleep apnea or other medical airway disorders. However, orthodontic treatment with airway in mind can often improve breathing, whereas orthodontic treatment that focuses on just aligning teeth can, in some cases, worsen existing breathing issues.
